About Us
The Coalition for Excellence in Value-Based Care at Home ("The Coalition for Excellence") is comprised of a group of organizations delivering healthcare services at home who are working together to promote policy change and adoption of value-based payments needed to distinguish high-quality providers of care in home- and community-based settings.
Now is the time to shape the future of care at home as patients age in place and healthcare organizations seek to absorb the lessons and experiences of the COVID-19 pandemic.
Ongoing Coalition Projects

Using the New York State VBP Roadmap and Managed Long-term Care Quality Incentive Improvement Program measures as a foundation, the Coalition for Excellence is creating a system that recognizes high-value providers of homecare.

The Coalition for Excellence is developing a proposal outlining an upside-only model test by which managed long-term care Medicaid cost-sharing payments in the NYS MLTC program will be put at risk for their corresponding acute care services. If services are avoided through support and coordination activities, the payments may be distributed to agencies as bonus payments. The demonstration would be budget neutral, with the option to fund it further.
The Coalition for Excellence is researching the current administrative process by which MLTC cost-sharing payments are paid and the authority needed to grant flexibility for MLTC cost-sharing payments to be put at risk for providers.

Prospective Members
Requirements
The prospective member’s business entity must be licensed by New York State as a Certified Home Health Agency (CHHA), Licensed Home Care Services Agency (LHCSA). They must be an innovator in the care at home space, such as a technology-enabled organization, or generally interested and aligned with the shared mission and principles of the VBC at Home Coalition.
The prospective entity must be geographically located, interested in expanding, or otherwise serves individuals in one or more geographic regions of New York State.
The prospective entity can be either not-for-profit or for-profit status.
